At the time of inspection the changing pad had a small rip in the water proof fabric. This was corrected when the changing mat was replaced.
Neglect occurred when two 2 year old children were found unsupervised in the parking lot and in a field next to the operation. Caregivers did not know the children have exited the playground. The children were unaccounted for approximately 5 minutes. The children were at risk of possible injury such as being hit by a vehicle, kidnapping, stray dogs, or falling in a ditch.

During the time of the incident the bungee cord used to secure the outdoor gate closed was never replaced after snapping due to weather elements. Which allowed the gate to be opened and easily accessible to children.
