Based on the information gathered, it was determined that the child was not supervised when the child opened the first aid kit and removed the scissors, and placed the scissors in the child's mouth. The child walked out to the playground and slid down the slide with the scissor still in the child's mouth before another caregiver on another playground noticed the scissors. Note: The rubber gloves and safety pins were also found on the playground from the same first aid kit.
Based on the information gathered the first aid kit was accessible to the children in a classroom that resulted in a child having access to open the kit.
