Through the investigation, it was determined that there were 24 children to one caregiver. The group's specified age was five years old. Making the group out of ratio by two children.

Two employee files reviewed did not have current SIDS and Shaken Baby Training.

At the time of inspection, there were six infants in the classroom with one caregiver, ages 12 to 17 months. This was corrected when one child unter17 months was moved for the infant room.

Two employee files reviewed did not contain current abuse / neglect training.

There were two broken boards on the playground accessible to the children in care.

At the time of inspection the playground surfacing was not maintained at its proper depth under and around the two playground play structures.

There were 17 broken pickets on the fence surrounding the playground. There was broken steering wheel on the car the child fell off of on the playground.

Based on the information gathered a child was injured when falling off a piece of playground equipment. The caregiver failed to provide first aid for the child that required medical treatment.

Based on the information gathered, the caregiver was not supervising the children on the playground when a child was injured and required medical treatment.

One van driver's transportation training was not current at the time of inspection.

The activity plan for the 12 to 24 month old class and the two year old classroom did not have current activity plans posted.

The gas test was expired.
