The operation has not conducted a fire drill since September 2023.

One of the operation's vehilces is not equipped with a first aid kit.

Two of the operation's vehicles used to transport children have electronic safety alarm systems installed that are not functioning.

The fire extinguisher located in one of the operation's vehicles has not been approved by the local fire marshal and is empty.

Based upon an investigation conducted by DFPS, a caregiver was observed physically redirecting children instead of using positive statements.

The monthly fire drill was not conducted for the month of June 2022.

This standard was evaluated during the investigation and was found to be deficient. The operation released a child to a person other than the child's parent without checking for or recording a copy the person's ID.

The last severe weather and lock down drills conducted at the center were in the months of July and April.

At the time of inspection 4 infants in care did not have a current infant feeding instruction form that had been completed by the parent. The latest instruction forms listed were for October.

At the time of inspection one child did not have his most current immunization record on file for review.

This standard was tasked over for this investigation and found to be deficient. It was determined that the center did not have copy of the child's medication prescription from a medical professional on file. Medication dosage was only administered by parent's instruction alone.

Two incidents were cut on video footage, in first case the caregiver pulled the child by the arm to set the child at the table, the second incident happened with other child, when child was running in the classroom, caregiver roughly grabbed the child by the top of the head to stop the child.
