Based on the DFPS investigation completed, it was found that the director was notified by the parents that the child required medical treatment as result of the incident that occurred at the operation and the director did not report this to Child Care Regulation.

I observed that the first aid and CPR is expired in 2 out of 7 personnel records evaluated.
I observed two small tree roots that present a tripping hazard on the playground.
I observed only one caregiver with a group of children at naptime that included three older infants.
I did not observe documentation of vision and hearing screening for two 5 yr. olds who have not yet started public school.
