The outdoor area contained numerous hazards during the walkthrough. A water hose was unraveled near the pre-k play area, which poses a risk as a tripping hazard. A wired gate contained a hole in the gated area, and also a wire that was unraveled, which poses a risk to children in care. The play structure that contained wood that had protruding nails from the wooden structure.

From 7AM-8:19AM children between the ages of 2-8 were observed watching television for over an hour during meal/breakfast time. After the school-age children left for the day, the television was still on, portraying Nickelodeon cartoons. The childcare facility did not have a lesson plan, and/or activities for the age group that was observed.
