During the inspection, the caregiver in the Primary classroom reported having 23 children in care. After conducting a count of children present in the room it was determined that there were 20 children present. This was corrected when the caregiver counted the number of children and confirmed only having 20 children in care.

Based on the information obtained during the DFPS investigation an incident report was not completed concerning a injury that occurred requiring medical treatment.

Based on the information obtained during the DFPS investigation there is sufficient evidence to support that the parents were not notified of an injury that occurred requiring medical attention.

There is not an annual fire inspection documented within the past year.

Based on the investigation findings, there is no evidence of an incident report for each minor injury.

An infant was missing enrollment information. This was corrected at inspection by the parent providing the completed enrollment paperwork.

Based on the investigation, a child not enrolled had been left unattended.

There were two staff members present at the operation with pending background checks and one staff member with an inactive background check.
