One child, 21 months old, was left unattended in the Grow Fit classroom for approximately 3-5 minutes. A staff retrieved the child when notified by another staff member. The child was returned to their primary classroom then after.

On 04/08/2025, one child was left unattended in the Grow Fit classroom for approximately 3-5 minutes. The person in charge, did not notify the parent of the incident immediately, and/or during pick-up the day of the incident. The following day, 04/09/2025, the parent was not notified of the situation until pick-up, at approximately 6-6:30PM, more than 12 hours after the incident occurrence.

On 08/08/2024 two infants, 12 months of age was grouped with school-age children, 4- 10 years of age. Based on the documentation observed the infants were present with the mixed group for approximately 10-15 minutes. A total of 18 children were signed in at one designated time. On 08/09/2024, one infant 12 months of age was grouped with school-age children, 4- 10 years of age. Based on the documentation observed the infant was present with the mixed group for approximately 20 minutes. A total of 15 children were signed in at one designated time.

One child's file did not include an immunization record for review. No record was in the child's file. Child's enrollment date is 10/04/2022.
One out of five infant bottles were not labeled. The bottle did not contain the child's first name, and/or last name. The operation labels the child's individual tray, but not the bottle. The operation has now made the proper changes to bottle labeling.
One caregiver was observed changing diapers within the infant classroom. The caregiver changed two diapers without washing their hands.
Based on the information obtained, there were reports of inappropriate discipline by the use of one's hand. Three out six children interviewed reported being hit by their caregiver.
The caregiver in the Toddler classroom was not aware of the specific ages of the youngest and oldest child in care. The caregiver in the Preschool classroom was not aware of the number of children in which they were responsible for. The caregiver stated there were a total of 8 children in care, and Licensing observed 11 children in care.
Based on the information obtained, three children slipped through the gate on the playground area. The caregiver was not near the children in order to prevent them from leaving the active play area. The lead teacher was informed by another caregiver in a separate area that two children were outside of the gate.
The double gates along the main street were broken which left an open space for children to have access to a main road. The double gates between the school-age active play area and toddler active play area contained a hole in the gate.
One out of ten children's records evaluated did not have an address listed for their emergency contact.
The first aid kits in both transportation vehicles were missing band-aids and a thermometer. Note- This was corrected during the inspection when the missing items were added to the first aid kits.
Three out of ten children's records evaluated did not have up to date immunization records on file.
The transportation binders did not include emergency medical authorization for the children being transported. Note- This was corrected during the inspection when the emergency medical authorization was added to the transportation binders.
The operation has not had an annual Sanitation Inspection since 10/28/2020.

Two infants who are not on table food did not have infant feeding instructions.
