One staff in Room 4 did not wash their hands after completing a diaper change of one child. The child was removed from the diaper changing mat and joined the remainder of the classroom. The staff proceeded to pick up another child with the same gloves to check the child's diaper for moist, and/or soiled. Shortly after the staff removed the gloves and placed them in the trashcan and joined the classroom. The staff did not wash their hands, and/or the child's hand after diaper changing task was complete.

The pre-k/schoolers playground area observed contained inadequate surfacing underneath the play structure. The green base of the play structure was torn and posed as a tripping hazard to children in care. The green base of the play structure also contained rocks underneath the ripped surfacing/padding, which was accessible to children in care.

Two staff that were present had expired CPR/First-aid certifications. One staff certification expired 06/19/2023. The second staff file observed expired, 06/12/2023.
Nap mats observed in Room 7 and Room 8 were not labeled in order to reflect the child that was sleeping on the nap mat during the inspection. The numbers on the cot did not reflect the child that was observed sleeping on the mat. Staff updated the mats by correcting the number chart to reflect the child present.
Two caregivers in Classrooms 3 and 4 were observed utilizing their personal cell phones while caring for children. Once the Director was informed of the cell phone use in the classroom, they were removed.
One caregiver was observed in Room 3 with 6 infants, 12-18 months of age. An additional caregiver was added to Room 3 to accommodate ratio. A total of 6 infants were observed, 11-18 months of age.
One child, four years of age was left unattended for approximately 2 minutes in Classroom 8 of the childcare center. Based on the information obtained the child was being transitioned from the classroom to the outdoor/active play area when the child was left behind during a transition period. The child was located in the classroom by a staff during a tour of the childcare facility. Based on the information obtained, a proper name to face check was not conducted to ensure all children were accounted for.
Three out of ten children records observed did not include a health statement. The health statement was not avaible in the child's file avaible for review.
The records were not avaible for review during the annual monitor inspection. Training certificates, and/or training topics were not avaible for review per the Health and Safety Audit.
The annual certification was dated April 8, 2021. The operation has not reviewed the current CPSC recall list to ensure the child care facility is free of unsafe products.
Based on the DFPS investigation, there is sufficient evidence to suggest that a caregiver did not demonstrate good judgement when the caregiver repeatedly handled the children in a rough manner, yelled at the children, and threw toys around the classroom.
Based on the DFPS investigation, there is sufficient evidence to suggest that the director did not ensure that Minimum Standards were maintained when allowing a caregiver to remain in the classroom after being aware that the caregiver was mishandling the children, yelling at the children, and throwing toys around the classroom.
13 incident reports were documented between October of 2021 to November of 2021 addressing concerns of excessive biting within the Toddler classroom. Based on interviews with parents the effect of children being bitten caused bruising, broken skin, and redness.
One caregiver in the Toddler classroom had a pending background check. Caregiver was included in ratio in a classroom with 2 caregivers, 12 children, 23 months- 2 years of age.
One caregiver was observed near the outside play area while two children were observed utilizing the bathroom inside of the classroom. The children could not be supervised properly without the caregiver being present in the bathroom, nor the classroom. The caregiver was outside was the children were in the restroom.
The day of the incident a total of 13 children were signed in, although a total of 16 children were observed on the video footage. Majority of the children were 4 years of age. There was no documentation for 09/09/2021 and 09/10/2021 for Room 8. The classroom has 16 children enrolled, in which 3 were not accounted for.
One caregiver was observed grabbing a child's left arm in an aggressive manner, and then placing the child on a carpeted area. The same child was later observed on video being grabbed by their right arm, and placed in a time-out area. The caregiver did not demonstrate good judgement, and/or self-control. The caregiver did not utilize patience when dealing with the child. A total of 16 children were observed in Room 8 with 1 caregiver. The caregiver did not have control of the classroom. There was no particular curriculum, or activity in place at the time.
Two infants, 4-6 months of age were observed sleeping in restrictive devices. One out of two caregivers were observed in front of the infants. Infants were removed from restrictive devices after instructed by Licensing.
One out of six staff records/file did not contain a CPR/First-Aid certificate available for review. There was no certificate in the staff/caregiver's file.
One out of five children records observed did not include the admission date. Several dates were observed throughout the admission forms. No date was listed for the actual enrollment, or admission date. Admission date was documented as 05/24/2021.
Two classrooms did not have an activity schedule/plan posted. The activity schedule for the Toddler age group was not available for review during the walkthrough. Activity schedule was observed on the web. Activity schedule will be posted.
