I evaluated 10 children's records. I observed the following: * Two were missing a date of admission** * One was missing the address for the elementary school** * Ten were missing information indicating whether the child has a special need. **NOTE: These two were corrected during the inspection.
Interviews were conducted and documentation of the incident was evaluated. It was determined that a toddler was left alone on the playground when the two caregivers who were working with the group did not follow the operation's procedure for transitioning children from one area to another. NOTE: Both caregivers were terminated after the incident and remaining staff were trained on new procedures for transitioning children.
