Based upon the information gathered during the investigation, sufficient evidence was available to support a child was left in the vehicle unattended

Based upon the information obtained during the investigation, a caregiver failed to follow proper procedures to ensure all children they were responsible for were accounted for. The caregiver called a child's name, another child responded the child was present, however, the caregiver failed to conduct a name-to-face to verify the child was present. Approximately 20-30 minutes later, it was discovered the child was not in the building due to being left on the center's vehicle.

Based upon the information gathered during the investigation, evidence was available to support the safety precautions for unloading children was not followed. As a result, a child was left on the van unattended for approximately 20-30 minutes.

Based on the documentation received, the transportation training certificate observed for the driver in question did not meet minimum standards trainer's qualifications.

Based off the information obtained from the investigation the operation failed to report that a child was left on the transportation vehicle unsupervised for an extended period of time.

A caregiver was allowed to have direct access to the children with an inactivated background check. *Note - This was corrected during the inspection when the caregiver was sent home until the results of a new background check is received.

The operation had not completed a Childrens Product Safety Annual Certification since 1/13/2020. *Note - This was corrected during the inspection when the Annual Certification was completed.

The operational policies did not have a policy for safe sleep for infants. *Note - This was corrected during the inspection when this policy was added to the operational policies.

There was no parent contact information and emergency contact information in the vehicles. *Note - This was corrected during the inspection when the parent contact information and the emergency contact information was added in the vehicles.

Six infant bottles were not properly labeled with the infant's first name and last initial. *Note - This was corrected during the inspection when the bottles were properly labeled.

The first aid kits in both of the transportation vehicles was missing antiseptic. *Note - This was corrected during the inspection when antiseptic was added to both first aid kits.
