Room C, the toddler classroom did not have the mats/cots labeled within the classroom. A total of 12 children were observed on mats that were not labeled, and/or labeled incorrectly. The operation will utilize a number chart to specify which cot/mat belongs to which child. The numbers were added to cots while Licensing was present.

Based upon the information obtained during the investigation, a caregiver failed to follow proper procedures to ensure all children they were responsible for were accounted for. The caregiver called a child's name, another child responded the child was present, however, the caregiver failed to conduct a name-to-face to verify the child was present. Approximately 20-30 minutes later, it was discovered the child was not in the building due to being left on the center's vehicle.
Based upon the information gathered during the investigation, evidence was available to support the safety precautions for unloading children was not followed. As a result, a child was left on the van unattended for approximately 20-30 minutes.
