An eight-year-old autistic child pulled the fire alarm on numerous occasions. The child was able to run out of one classroom unsupervised and into another room to pull the fire alarm.
One staff observed as kitchen staff was present at the childcare facility with a pending background check. The first date of employment for the individual is May 30, 2023. Staff was removed from care on 06/01/2023.
The ceiling in the pre-k classroom had 7 panels that contains holes, and water stains. The schoolers classroom contains a hole in the bathroom door, as well as the side of the handwashing sink.

Based on the information obtained, there was no documentation available to conclude the Director's presence at the childcare facility. There was no timecard/timesheet available for review.
Two children were observed inside the classroom during active play time. Shortly after, the assistant Director and the caregiver was observed indoors while the remaining of the children was outdoors unsupervised. During naptime, one caregiver walked out of the classroom into the hallway, leaving children unattended.
The Director is allowing staff to be present with pending background checks. The operation is experiencing repetitive deficiencies related to background checks.

Upon arrival a total of 2-3 caregivers were present within 2 classrooms. There was no one to assist Licensing during the follow-up inspection from 7:29AM-8:45AM. Licensing was performing caregiver duties. The assistant Director arrived at 8:45AM and assisted within the classroom.

A walkthrough was conducted on 05/12/2023, in which a total of 7 infants/children were observed, 3-22 months of age. The youngest child observed was approximately 19 months younger than the oldest child in the infant classroom.
Based on the walkthrough on 05/12/2023, one staff member was observed with 6 infants, 3-22 months of age. The specified age group is 0-17 months of age. Another staff member was then informed to join the infant age group to accommodate ratio.
One staff did not receive or did not acknowledge receipt of training orientation. There was no documentation in the staff file to conclude orientation was received. Form was provided to staff while Licensing was present. Form was reviewed and signed by staff.
