Two staff CPR/First-Aid certification did not adhere to the guidelines of skill-based training or consist of a curriculum that incudes use of a CPR manikin.

Four out of five children records observed were not completed with a parent signature. The admission forms were missing various signatures/completions throughout the 2935 form.

A water hose was observed on the outdoor area unraveled, and accessible to children in care.

A follow-up inspection was conducted on 2/10/2025, in which the childcare had not obtained a current health statement for one child. One child enrolled 07/30/2024 did not have a health statement on file for Licensing staff to review.

Two staff in the Little Genius classroom did not know the ages of the children, in which they were responsible for.

A first-aid kit was not observed in the staff personal vehicle that was utilized to transport a child from school.

One infant, 4 months of age was observed in a walker in the Infant 1 classroom. The walker was removed from the infant room/childcare facility while Licensing was present.

The playground area contained broken pieces and ledges that poses a risk to children in care. The walking area on the play structure contained broken wood/boards that were cracked. A water hose was observed on the playground, which posed as a risk to children enrolled.

As of 08//22/2024, the previous staff/Director background check was not inactivated within the 7-day timeframe.

Eleven children observed in the Pre-K Pirates classroom were placed on various nap mats. Nap mats observed contained incorrect children's names, and/or no name.

One infant bottle observed in Infant 1 classroom was not labeled with the child's first and last name. The bottle was labeled with the child's first and last name.

The most recent fire inspection was dated 06/12/2023.

The childcare facility contained tiles that were hanging in the school-age classroom, which poses a risk to children in care. Several tiles in the school-age room and toddler classroom contained water stains. The AC vent observed in the school-age classroom did not contain a cover which caused for the AC filter to be exposed to children in care.

Infant feeding instructions for two infants, 3-4 months of age did not have infant feeding instructions available for review. The most recent infant feeding instructions observed were dated October 2023.

One child enrolled January 23, 2024, did not have a health statement available for review within one week of enrollment.

The staff present observed with the children in the infant classroom was not aware of their ages. Only three children were observed in care.

Based on the menu observed, the childcare facility is not rotating, and/or substituting the items served to children in care. Children in care had cereal and milk for breakfast on 11/02/2023 and 11/03/2023. Menu modifications were made while Licensing staff was present.

Based on the information obtained, two staff were present on 10/21/2023 without an active background check. Background checks for both staff were submitted on 10/21/2023. Staff are no longer employed at the childcare center.

Two caregivers did not have the aquired trainings for a child with a special needs.

The playground area had two logs with nails sticking out. This was corrected at inspection when the director removed the logs from the area.

The playground area had two round pools and a children bucket that was filled with water and assessable to children. This was corrected at inspection when the director emptied the water and removed the pool.

Based on the information obtained, the special needs training certificate observed did not include the date of training, the total number of hours acquired, and/or the trainer qualifications.

An eight-year-old autistic child pulled the fire alarm on numerous occasions. The child was able to run out of one classroom unsupervised and into another room to pull the fire alarm.

One staff observed as kitchen staff was present at the childcare facility with a pending background check. The first date of employment for the individual is May 30, 2023. Staff was removed from care on 06/01/2023.

The ceiling in the pre-k classroom had 7 panels that contains holes, and water stains. The schoolers classroom contains a hole in the bathroom door, as well as the side of the handwashing sink.

The fire inspection was dated 04/21/2022.

Three infants, 12-14 months of age was observed not strapped into their seats during mealtime. Licensing assisted with strapping the infants in for mealtime.

The most recent sanitation inspection report was dated 04/12/2022.

One caregiver in the pre-k classroom was observed utilizing their personal cell phone during children naptime.

One caregiver observed in the infant classroom changed two infant's diapers without washing their hands. The caregiver was informed of proper handwashing procedures, in which the staff washed their hands after being informed.

During the absence of the designated Director, the assistant Director was not able to locate children's file who was present, locate the annual fire and health inspection, as well as provide guidance on staff employment, and/or transportation services. The operation received several deficiencies related to supervision concerns, and repeated personnel deficiencies.

One infant, 12 months old did not have an admission/enrollment form available for review.

Two infants' hands were not washed after diaper changing procedures.

Four infants (12-17 months) observed in the Little Seeds classroom was observed with large food items on their plate. The food items served includes, whole fruit pieces, a burger sliced in half, and green beans.

One infant, 3 months of age was observed in a bouncer seat for approximately 30 minutes. The caregiver was informed of the danger, in which the infant was removed.

One child, 2 years of age observed in the Tiny Giants classroom was placed in a highchair, not strapped in.

The most recent smoke detection system was tested/dated 03/09/2022.

Lysol and OdorBan was observed in the Pre-K pirates classroom a shelf and near the handwashing sink; the cleaning supplies were accessible to children in care.

Based on the information obtained, there was no documentation available to conclude the Director's presence at the childcare facility. There was no timecard/timesheet available for review.

Two children were observed inside the classroom during active play time. Shortly after, the assistant Director and the caregiver was observed indoors while the remaining of the children was outdoors unsupervised. During naptime, one caregiver walked out of the classroom into the hallway, leaving children unattended.

The Director is allowing staff to be present with pending background checks. The operation is experiencing repetitive deficiencies related to background checks.

One staff providing transportation training did not obtain transportation safety training prior to transporting children.

Two caregivers in the infant classroom were not aware of the name of an individual present in their care. Based on the information obtained the child started 05/11/2023 within the Toddler classroom. Today the child was placed in the infant classroom.

Upon arrival a total of 2-3 caregivers were present within 2 classrooms. There was no one to assist Licensing during the follow-up inspection from 7:29AM-8:45AM. Licensing was performing caregiver duties. The assistant Director arrived at 8:45AM and assisted within the classroom.

One staff was observed transporting children, and present caring for children in the Pre-K classroom. The current background check status is pending for the individual.

One caregiver observed in the Toddler classroom did not have a current CPR/First-aid certification prior to having unsupervised access to children in care.

The annual children's product certification was dated, 1/17/2022. A 2885 Form was provided to the Director in order to ensure an updated copy/review was obtained.

One caregiver in the Pre-K classroom was observed utilizing their personal cell phone while caring for children. Several children were observed napping, while other children were observed during bathroom breaks.

A total of 21 children were signed in on the sign-in/sign-out log, while a total of 34 children was observed in care. The sign-in log did not reflect the total number of children present in care.

One staff member observed in the Tiny Giants (Toddler) classroom did not have an active background check available for review. The individual observed had a pending background check and was observed caring for 4 children, 18-20 months of age.

A walkthrough was conducted on 05/12/2023, in which a total of 7 infants/children were observed, 3-22 months of age. The youngest child observed was approximately 19 months younger than the oldest child in the infant classroom.

Based on the walkthrough on 05/12/2023, one staff member was observed with 6 infants, 3-22 months of age. The specified age group is 0-17 months of age. Another staff member was then informed to join the infant age group to accommodate ratio.

One staff did not receive or did not acknowledge receipt of training orientation. There was no documentation in the staff file to conclude orientation was received. Form was provided to staff while Licensing was present. Form was reviewed and signed by staff.

One caregiver observed with the Toddler age group did not have a active background check. The individual was the only caregiver observed in the classroom. The individual hire date was documented as 09/12/2022.

The Toddler and the school-age classroom were observed with cots, and/or mats that were not labeled. The caregivers present could not specify which cot belonged to which child.

A total of 14 children were signed in on the sign-in/sign-out log, while a total of 27 children were observed in care. The sign-in log did not reflect the total number of children present in care.

The operation has not had a fire inspection conducted since 9/2/2020.

Children who are 18-23 months of age were allowed to watch the television. Note- This was corrected during the inspection when the television was turned off.

A six-month-old infant was asleep in a bouncer which is considered a restrictive device. Note- This was corrected during the inspection when the infant was removed from the bouncer and laid in a crib.

Three out of ten children's records evaluated did not have up-to-date immunization reccords on file.

Two climbing structures did not have sufficient loose-fill under and surrounding the structure. One climbing structure that requires loose-fill was placed on grass.

The fire extinguisher was placed on the floor within the walkway of the transportation vehicle. The fire extinguisher was not properly installed.

One out two transportation vehicles did not have an electronic child safety alarm installed on the bus. The operation did not have the manufacturers instructions to ensure the alarm system was installed properly.

The Toddler and Two-year old classroom contained chipped paint in various areas of the classroom. The changing table, the exit door, and wall area contained chipped paint.

The child safety alarm installed in the transportation vehicle was not in working condition. The alarm did not alert/alarm the driver to inspect the vehicle.
