Based on information received from DFPS several children did not wash their hands after a caregiver changed their diapers.

Based on the findings of a DFPS investigation, a caregiver did not intervene when necessary to ensure the safety of children in the classroom.

A child was left alone unsupervised in a classroom when the class transitioned from the classroom to the cafe for lunch. The child was alone for approximately seven minutes.

I observed a bottle of children's Benadryl that expired in October of 2022. NOTE: This was corrected during the inspection.

I did not observe documentation that the in-person portion for CPR was completed.

Interviews were conducted and procedures were evaluated. It was determined that the operation did not report to Child Care Regulation an incident involving a 4 yr. old running out of the classroom and the operation.

The operation did not report the suspected abuse of a child to HHSC - Child Care Regulation. The operation was aware of the incident where a caregiver placed a bib over the face of the infant.

During a recent A/N investigation, it was determined that a caregiver placed a bib over the face of a crying infant.

During an A/N investigation, it was determined that a caregiver became frustrated with an infant child and used profane language.

When asked how many children were in the group, a caregiver for school-age children didn't know the correct number.

I observed a caregiver supervising 16 three and four yr. old children. The median age of the group was 3 years and the required child/caregiver ratio for three year olds is 1 caregiver with 15 children. NOTE: This was corrected during the inspection.

During the walk-through inspection, a caregiver stated that there were 40 children in the group, but when I counted, there were only 35.

Interviews were conducted and operation procedures were evaluated. It was determined that a preschool child was left alone on the playground when the class transitioned back inside to the classroom.

Interviews were conducted and surveillance tape was reviewed. It was determined that a caregiver placed two 3 yr. old children in the restroom during naptime as a disciplinary measure. NOTE: The caregiver was terminated.

During an A/N investigation conducted by DFPS, it was determined that a 21 month old toddler left the playground through a gate that was not properly secured and was found in the parking lot of the operation. It was determined that the gate was not checked by the caregivers upon entering the playground with the group of children. It was also determined that the caregivers were sitting and could not see all areas of the playground. It was also determined that the caregivers did not account for the children in their group and were not aware that the child was no longer with the group.

Interviews were conducted and the operation's emergency procedures were evaluated. It was determined that a 6 month old infant was left in the classroom without adult supervision during a fire drill.

Caregivers were interviewed and procedures were evaluated. It was determined that an infant child was left in the building during a fire drill due to caregivers not verifying the children in the group before exiting the building. During the inspection today, I asked a caregiver how many children in her room and she said 15 children but there were 16 children in the group.

Interviews were conducted and procedures were evaluated. It was determined that a preschool-age child was left alone in a classroom when a caregiver did not follow the operation's procedure for transitioning children.
