Based on the findings of a DFPS investigation, a caregiver did not intervene when necessary to ensure the safety of children in the classroom.
A child was left alone unsupervised in a classroom when the class transitioned from the classroom to the cafe for lunch. The child was alone for approximately seven minutes.

Interviews were conducted and procedures were evaluated. It was determined that the operation did not report to Child Care Regulation an incident involving a 4 yr. old running out of the classroom and the operation.

During the walk-through inspection, a caregiver stated that there were 40 children in the group, but when I counted, there were only 35.

During an A/N investigation conducted by DFPS, it was determined that a 21 month old toddler left the playground through a gate that was not properly secured and was found in the parking lot of the operation. It was determined that the gate was not checked by the caregivers upon entering the playground with the group of children. It was also determined that the caregivers were sitting and could not see all areas of the playground. It was also determined that the caregivers did not account for the children in their group and were not aware that the child was no longer with the group.
Interviews were conducted and the operation's emergency procedures were evaluated. It was determined that a 6 month old infant was left in the classroom without adult supervision during a fire drill.
Caregivers were interviewed and procedures were evaluated. It was determined that an infant child was left in the building during a fire drill due to caregivers not verifying the children in the group before exiting the building. During the inspection today, I asked a caregiver how many children in her room and she said 15 children but there were 16 children in the group.
