One staff CPR/First-Aid certification did not adhere to the guidelines of skill-based training or consist of a curriculum that includes use of a CPR manikin.

One child, in the Toddlers classroom was bitten on numerous occasions. Based on the video footage observed, the caregiver in charge was observed conducting diaper changing in the bathroom while one child was bit and hit on their head consecutively by another child in the classroom. There was a total of four children outside of the bathroom area, where the staff did not have visual awareness to properly supervise the age group, in order to prevent the incident, or intervene immediately. As a result of the bite incident, the child sustained a break in the skin.

Two caregivers were observed with 12 infants, 4 months-15 months of age. Shortly after, a third caregiver was added, which concluded three staff with 12 infants, 4-15 months of age. The age groups were separated amongst two classrooms. Classroom A contained 9 infants with 2 caregivers, while Infant B classroom contained one caregiver with 3 infants.

One child, two years of age was left unattended in the Toddler classroom for approximately 4-5 minutes. Based on the footage observed, the classroom was transitioning from the Toddler classroom to the Enrichment classroom when the child was left behind. The child was observed by another parent/guardian who has a child enrolled at the childcare center. Based on the information obtained, a name to face check was not conducted to ensure all children were accounted for.

This standard was evaluated in regard to a report received and compliance was not met. The center is not maintaining compliance with child/staff ratios. Classroom Infant B was out of compliance with child/staff ratios on the following days: 8/20/24, 8/21/24, 8/26/24, 8/27/24, 8/28/24, 8/29/24, 8/30/24, 9/5/25, 9/11/24, 9/25/24 and 10/2/24. There are 6 to 7 children, ages 11-17 months old, with one caregiver during naptime. The specified age group is 12-17 months old and one caregiver can only care for five children.

The diaper changing mat in the infant B and the Toddler classroom was torn apart to the extent of exposing the foam surfacing that children are placed on. The diaper changing mats cannot be properly sanitized due to the exposure of surfacing material.

The caregiver did not utilize good judgment by grabbing a child's arm in attempt to redirect the child to pick up toys off the carpeted area. Based on the video footage observed, the caregiver was observed grabbing the child by the arm, and then further pulling the child's arm to get the child to pick up toys. The caregiver chased after the child, and then continued to hold the child by the arm until toys were picked up completely.

Two caregivers were observed in Infant B classroom with 14 infants, 13months-18months of age. The oldest child in care was moved to the Toddler classroom.
One caregiver was observed placing a child in a chair in between a table and a shelf, in attempt to prevent the child from disturbing others during naptime. As the child was attempting to get out of the chair, the caregiver continued to place the child in the seat. Based on the video footage observed, the caregiver did not utilize good judgment by insisting the child sit down into their seat, even after the child cried.
Two out of ten staff records observed only included an online CPR/First-Aid certification. There was no documentation to support in-person training, and/or hands-on skill-based instructions, and/or practice.
A sanitation inspection had not been conducted. The operation is scheduled for a health/sanitation inspection on 07/22/2022.
