The caregiver in suite 200 cleaned a child nose and did not wash her hands afterwards. Note: When pointed out the caregiver washed their hands.

Based on the information obtained, including video footage, the caregiver was not practicing positive methods of discipline and guidance when picking up the children and roughly placing the children in chairs.

In the pre K classroom being used by the school age children there was no soap at the sinks for the children to wash their hands. Note: Soap was added during the inspection.

The center did not report cases of COVID19 to HHSC.

In the school age classroom one caregiver stated that there were 35 children and the other caregiver stated that there were 34 children. There were actually 33 children in care.

One staff, alone with the children, had completed the cpr training on line and did not attend a class.

One child's record was not up to date as they did not show the child had received the MMR immunization.

Based on the information obtained during the investigation there is sufficient evidence to support that the children were not being supervised when a caregiver stepped out of the classroom to take out the trash and also was sweeping the floor while children were in care and one child climbed onto the table.
