While evaluating children's records, I observed the following deficiencies: * No documentation of a food allergy emergency plan for a child with a documented food allergy. * Complete contact information for the child's doctor documented in 3 out of 4 children's records. *No documentation of names and/or phone numbers for persons authorized to pick up the children in 3 out of 4 children's records evaluated. *No documentation of an immunization record in 2 out of 4 children's records evaluated.
During the inspection, I observed the door to the staff restroom open. In the restroom, on a low shelf, I observed cleaning products that were accessible to children.
I did not observe a thermometer in the first aid kit. NOTE: This was corrected during the inspection.
