I observed that the first aid kit did not include cotton balls.

I did not observe an activity schedule posted at the center. NOTE: This was corrected during the inspection.

I observed a child being served snack without his hands being washed. NOTE: This was corrected during the inspection.

The number of employees who had left their employment in 2021 was not reported to HHSC - Child Care Regulation by the 1-15-22 deadline. NOTE: This deficiency has been corrected.

Neither one of the caregivers knew how many children were in care when asked.

On several occasions during the inspection, both caregivers were on one side of the center and there were children in other areas of the center where the caregivers could not see them.

While evaluating children's records, I observed the following deficiencies: * No documentation of a food allergy emergency plan for a child with a documented food allergy. * Complete contact information for the child's doctor documented in 3 out of 4 children's records. *No documentation of names and/or phone numbers for persons authorized to pick up the children in 3 out of 4 children's records evaluated. *No documentation of an immunization record in 2 out of 4 children's records evaluated.

During the inspection, I observed the door to the staff restroom open. In the restroom, on a low shelf, I observed cleaning products that were accessible to children.

I did not observe a thermometer in the first aid kit. NOTE: This was corrected during the inspection.

I did not observe the center's procedures for staff vaccines documented in the center's operational policies. NOTE: This was corrected during the inspection.
