During a walk through of the operation it was observed that there were three holes in the wall, the trash can was overflowing with trash and onto the floor, the bathroom floor and classroom floor had several pieces of paper on it.

The operation was unable to provide a building and grounds checklist.
The operation did not have a current employee list posted for review. The employee list posted had caregivers that were no longer employed at the operation. This was corrected at inspection when the caregiver posted an updated employee list.
The operation was unable to provide documentation for a fire drill, lock-down drill for a volatile or endangering person or sheltering drills.
The operation was only able to provide documentation of CPR/first aid training for 3 out of the 13 currently employed caregivers.
A front desk administrator/ staff present at the time of inspection did not have an active background check at the time of the inspection.
A newly hired caregiver present at the time of inspection and counted in the infant child to caregiver ratio did not have safe sleep, SIDS or shaken baby training.
The operation was unable to provide written feeding instructions reviewed within the last 30 days for any of the 7 infants present at the time of inspection not yet on table food.
The epinephrine injection for a child in care with a known food allergy had expired on 9/24/2022 and was still on file at the operation. The operation also had expired anti static valved holding chamber as of 2/18/22 and albuterol as of 5/2023 for a child in care.
During the walk through of the operation the walls were observed to be written on, the wall trimmings were cracked with chipped paint,. The toys were broken on the playgrounds presenting risk of cuts, scrapes and entrapment, the playground had trash accessible to children in two seperate areas, due to recent rain prior to my arrival there were bodies of water in buckets, toys and structures, two silver structures were observed to be broken and placed in the center of the usage zones and the outdoor sink was filled with surfacing gravel rocks. The bathrooms within the classrooms had urine and feces requiring staff to walk around the center and flush all toilets, the bathrooms also had toilet paper on floors, potty trainings seats on the floors, a thick brown muddy substance cracked along the drainage hole in the toddler classroom, exposed toilet paper handles due to the covering on the san jamer holder being broken presenting risk of entanglement. The handwashing sinks were covered in brown stains. The window frames had broken window screens and the inside of the frames were filled with dust and dirt. The toys and equipment in the classrooms for learning were covered in crayon and colorful markings and the floors in the classrooms had small random items resembling trash on the floors presenting choking hazards.
A child was left unattended on the playground without direct supervision for hours.
School age caregiver did not know how many children caregiver was resposnbile for. Caregiver stated 26 when there wsa 27 children in care.
During the walkthrogh it was found that the school age room was out of ratio by one child. Note: This child was placed in a different classroom.
It was found that caregiver did not report to a parent about a less serious injury during pick up. A child was hit on the head with a block, and this was not reported to parent by the caregiver. Note: The caregiver has been terminated.
Based on information gathered from the investigation, it was found that a caregiver used inappropriate discipline practices on a child.
Based on the investigation, positive methods of discipline were not shown by a caregiver.
The CPSC posting was not signed within the past year. This was corrected during the inspection.
There were no feeding instructions for infants not on table food.
There is not a fire inspection documented for the past year.
