During the initial walk-through, two caregivers gave the incorrect number when asked how many children were in the group.
I reviewed the center's transportation binders. I did not observe documentation of emergency contact information for some of the children on the list.
Ten children's enrollment records were evaluated. The following ws observed: 1. One child'd file was missing documentaiton that immunizations are current. 2. One child's file was missing documentation of vision and hearing screening.
I observed the infant feeding instructions had not been updated in the last 30 days for 6 infants who are not yet eating table food.
A caregiver who was counted in ratio walked out of the classroom to put away cots after naptime, leaving one caregiver with 10 older infants. The required/childcare ratio for older infants is 1 caregiver to every 5 infants.
I observed a caregiver in the young infant classroom change a child's diaper without washing the child's hands afterwards.
I observed that there were not cotton balls in the first aid kit.
I did not observe the policy concerning the benifit of activity included in the center's operational policies.
I observed that the transportation safety training for an employee who transports children expired in August of 2021. NOTE: This was corrected during the inspection.
I observed that the food allergy emergency action plan was not signed by the parent.
I did not observe documentation of pre-service training in 1 out of 5 personnel records evaluated.
I observed that the first aid kit on one of the busses did not have tweezers. NOTE: This was corrected during the inspection.
I observed one caregiver supervising 16 three and four yr. old children. The median age of the class is 3 years and the required child/caregiver ratio for 3 yr. olds is one caregiver with 15 children. I observed one caregiver supervising a mixed age group of toddlers and 2 yr. olds. The median age of the group was 18-to 23 monthd and the required child/caregiver ratio for toddlers is 1 caregiver to 9 children. NOTE: This was corrected during the inspection.
