Four child files were reviewed at inspection. Two children files were missing the emergency contact address for the individual listed by the child's parent. Two children files were missing the parent signature acknowledging the enrollment information provided and one of those files did not have the section for authorization for emergency medical attention, the additional information section providing information of the child's food allergy, and did not have the health statement section of what the parent will do regarding the health statement for their child.

The operation decreased the size of the outdoor play area used by the children in care prior to notifying to Child Care Regulation.
The use zone for a swing observed being used by children did not meet the required use zone as it did not extend twice the height of the vertical distance from the swing beam to the surface below and only measured about 2-3 feet away from the fence. Additionally, a stationary equipment with a slide attached was observed without the necessary use zone of 6 feet all around.
Five children did not have cots, nap mats, or bed to sleep in and were laying on blankets folded in half.
