Two operational staff did not know how many children they were responsible for or know the ages of the children inside their respective classroom.

Three operational staff were observed without a clear background check. The first classroom had 1 operational staff and 4 children. The teacher was observed teaching children how to do mathematics inside the classroom. The second classroom 2 operational staff and 4 children. The teacher was observed teaching children how to do mathematics inside the classroom.

One volunteer was observed in the school age classroom with the present of children.

The operation has not complete their annual children product certification since 2021.

The operational staff was not able to produce the timesheet for the director to vertified the present of her operation hours of a minimum of 30 hours per week.

Three children were observed playing with their Ipad during the inspection.

A lock down drill that was conducted in July 2022 had not been documented. Note: The provider documented the date and time of the lock down drill that was conducted.

A nap cot was placed directly in front of a door entry to the classroom. Note: The provider moved the nap cot to provide a clear pathway to the room.

1 out of 9 staff files evaluated did not have documentation of training in the following topics: 1. Handling, Storing and Disposing of Hazardous Materials. 2. Responding to Food Allergies 3. Understanding Building and Physical Premises Safety 4. Emergency Preparedness 5. Preventing the Spread of Communicable Diseases.

A yellow and white tube of disinfectant wipes was observed in an unlocked cabinet in room 9 which was accessible to children during the inspection. Note: The provider removed the wipes during the inspection.

Steps on play structure are rusting and beginning to have part of step fall. The area is approximately 3 x 2 inches. This section is lower than the rest of the step, and the rusty, metal edge is showing. Note: Photos were taken.

5 out of 7 employees do not have valid CPR/FA training. Training was completed in an online course without the use of a CPR manikin.

Three employee's files were reviewed and missing 24 hours of required training.

Two caregivers observed with children did not have a background check on file.

Two caregivers conducting transportation did not have up to date training on file.

During inspection, 3 different teachers were yelling at one child, threateing to tell parents of their behavior. One caregiver asked for a belt from another caregiver to threaten the child into behaving.

2 cargivers have expired first aid and CPR training. This training expired on April 23, 2021.

Diaper changing mat in toddler room had a tear approximately 1 inch long. Note: Photos were taken.

3 out of 10 children's admission forms were missing the designated emergency contact's address.
