One child was missing the health statement in the file.

One child was missing the address for the emergency contact information.

Caregivers could not access to children's records upon requested of Licensing. All the files were in the locked office and could be unlocked by the director. Note : corrected. The director came and showed all the files to Licensing. She has one spare key to unlock her office for caregiver can access to children file in case emergency and/or upon request from Licensing.

The center allows parents to pick up/drop off/visit their children from lobby. The parents were unable to come inside the classrooms. The mandatory policy for not entering the facility during the COVID 19 pandemic expired on 04/25/2021.

One driver does not have current two hours of annual transportation safety training.

The driver had incorrect dates, time and name on the list of children being transported.

Based on the information obtained during the investigation, there is sufficient evidence to support that the driver failed to exercise good judgment. The driver did not see the child before she picked the child up for the first time. She did not recognize that they put the wrong child in her vehicle for picking up.

It was noticed that the emergency medical information for one child being transported to the center was not available on the van. NOTE: this was corrected at inspection. The staff made copy of the enrollment form and put it in the binder.
