An electric outlet was observed to be exposed in the ground surface near electric outlet near the school age playground.

One operational staff was observed inside the classroom without a clear background check. There was a total of two teachers and thirteen children. The age of the children are two years old to three years old. The children were sleeping during the inspection.

Children were observed sleeping ono the floor with no nap cot or mat. Note: This was corrected during the inspection when the caregivers put the children on their assigned cots.

The diaper changing mat in the infant room is torn in two places and cannot be cleaned properly.

The feeding instructions for 4 infants had not been signed and dated by parents within the last 30 days.

The loose-fill material on the playground is inadequate for the height of one of the play structures. The platform of the play structure is over 5 ft high and there is only 6 inches of loose-fill material. An additional 3 inches is required.

There were infants observed sleeping in their cribs with pacifiers that had a strap connecting to their clothing. Note: The pacifiers were removed during the inspection.

Cleaning supplies were stored in an unlocked cabinet in a classroom with children present. The handle to attach a lock on the cabinet was broken and cannot be used properly. Note: This was corrected during the inspection when the lock was placed over the cabinet.

The button to the child safety alarm in the van is broken and cannot be used properly.

The use zone on the sides of the bucket swings does not extend at least six feet. An additional 3 1/12 feet is required on both sides of the swing set.

The use zone for bucket swings in the front and rear do not extend at least two times the vertical distance from the swing beam to the top of the swing- sitting surface. An additional 6-7 feet is need in the front and rear.

There is a quarter size hole with 6 smaller pen size holes in a ceiling tile above the door to the girls bathroom in the toddler room. Holes are large enough for pests or attic debris to fall through into classroom. Note: Photos were taken.

Fire drill for May was not conducted.

Prek play structure has a bolt that is not secure making a bracket stick out from the structure approximately half an inch. The bracket is approximatly 3 feet off the ground. This could catch a child's clothing causing choking. Photos were taken.

Carbon monoxide detector was not tested in May 2021.

Smoke detectors were not tested in May 2021.

During inspection there were seven areas on play structures for children that exposed sharp, rusty, edges that could pinch or injure a child. The toddler play structure had 4 areas that were rusty and sharp. First area is approximately 1 inch long located under straight slide located on first level platform off the ground. Second area is approximately 6 inches long located on first platform level of black up and down climbing area. Third area is approximately 6 inches long diagonally shaped also located on the black up and down climbing area. This are appears to be rusting under the black coating. Fourth area is located on second platform area in front right corner of play structure. The area is approximately 3 inches wide and 2 inches tall. This is where the platform is bolted to the structure. It is also rusting under the black coating and appears to be painted over before. Infant play structure had 3 areas that were rusty and sharp. First area was located on the first step approximately onei inch off the ground. The rust exposed area is at least 12 inches wide. The black coating has worn off in this area exposing the metal. Second area is located on the second step at the edge of the step. The expanded metal is exposed from the black coating wearing off. At this time the sharp area is approximately 1 inch wide. Third area is located where the slide is. This area is 7 inches wide at the edge of the platform. This is where the infants sit to slide down. The exposed metal is large enough that pieces of mulch are stuck between the black coating and the metal.

Fire extinguisher was not inspected in May 2021.

Diaper changing mat in infant room had one cut in the plastic approximately 1 inch long. The diaper changing mat in the toddler room had two cuts that were approxiamtely 1 inch long. Note: Photos were taken.
