One caregiver requires three hours of annual training. One caregiver is lacking annual abuse/neglect training.

The infant group with a specified age group of 0-11 months contained 11 children with three caregivers. The group was over ratio by one infant.

The Operational Policies are lacking a physical activity policy.

Parent contact information for one child and medical treatment authorization for three children transported at inspection were not available during transport. Note: parent contact and medical treatment authorizations were added to the transportation binder at inspection.

Two infant classrooms were observed over ratio at inspection. Infant group with children of age 3m-8m contained five infants; the group was exceeded by one child. Infant group with children of age 11m-13m contained six infants; the group was exceeded by two children.

Parents of all enrolled children have not been notified of the ability to access the building. Note: the operation posted a notification informing parents that they may enter the operation during the inspection.

An eleven month old infant was observed napping with a small blanket/stuffed animal attachment.

A four month old infant was observed covered by a blanket while napping in a crib.

Parents are prevented from entering the facility.

Screen time activity was observed for a group of children 18 months- 2 years; the group contained five children below the age of two. Note: the caregiver was prompted to end the activity and incorporate an age appropriate morning activity for the group.

Eight hours of annual training certificates for the director have been provided to date.
