During the inspection numerous hazards were observed. Ceiling tiles located in the classrooms, dining area, hallway, and bathroom had water damage and/or the ceiling tile was not intact. Numerous tiles were missing from the wall in the bathroom children in care utilize, exposing them to debris and unknown substances on the wall.
A large plastic tub of standing gray water was observed under a sink in the bathroom utilized by children in care. The washer back flowed into the children's sink in the bathroom, causing concern that the children in care had access to contaminated gray water in conjunction with standing water. This was immediately corrected during the inspection when the tub of water was emptied and the children in care no longer had access to the bathroom temporarily.
Two shelves in the infant classroom presented a tipping hazard should the infants in care pull-up on the shelving. The infants in care had access to the shelves as the shelves had toys on them for the infants in care.
A caregiver in the infant room was observed using a cell phone while supervising children in care. This was corrected at inspection when the caregiver put the phone away.
Through the course of the investigation, it was determined prohibited punishment was used in the form of caregivers yelling at children in care.
A new caregiver was lacking training certificates to verify her pre service requirements. Various other caregivers were lacking training certificates in their files to verify annual training hours.
