Information obtained during the investigation shows that a child was left unsurpervised in a classroom.

One caregiver was present with an inactive status on their background check. This was corrected at inspection when the caregiver left.

One caregiver did not know how many children were present in the classroom.

It was found during a self-reported investigation that a child was left unsupervised on the playground for approximately one minute.

A mop bucket with dirty mop water was observed next to a toilet in a bathroom used by children. This was corrected at inspection when the mop bucket was removed.

During an investigation, it was found that children under the age of three years old used items labeled for use for children three years and older. This was found corrected during the investigation inspection, when it was observed that the items had been removed from the classrooms with children under the age of three.

During an investigation it was found that a child was not supervised appropriately when the child gained access to an item that was not labeled for the child's age group.

The operation did not report to Licensing within the required 2 days when they were made aware a child required treatment from a medical professional after being in care. This was corrected at inspection when the operation stated their plan to notify licensing within the required time frame when they are notified a child receives medical treatment related to incidents in care.

The slide on a hill that's being used on the playground is not marked off 6 feet around has no surfacing material to prevent fall injuries. There were also gaps around the slide area that are tripping and entanglement hazards.

The operation has not obtained the gas inspection every 2 years as required.

The operation did not have written feeding instruction for 2 infants. The feeding instructions of 3 children did not indicate the number of ounces to feed the children.

The operation has not obtained the fire inspection for 2021, as the last fire inspection was conducted on 11/16/2020.
