Observed a broken toy with sharp edges outside on the playground and a broken toilet seat in one of the bathrooms. The broken toy was removed from the playground area.

Observed 3 classrooms who did not know how many children they were caring for. One caregiver had to count the kids to know how many they had.. In another classroom two caregivers stated that they had 6 children but had 5 children. In a third classroom two caregivers did not know how many children they had. One caregiver stated that there were 6 kids, the other teacher said 8 kids and there were only 7 children in the room.

An infant was observed to be asleep in their crib with a bib around their neck. This was corrected when the caregiver removed the bib.

Three classrooms had pealing drywall and had baseboards that were falling or missing. There was an outdoor playground that is not currently being used but is part of the capacity that had piles of items that were needing to be thrown away like old toys. There were also metal steaks that hold the border of the playground together that were sticking up and could be a tripping hazard. I spoke with the director regarding these concerns.

On camera I Observed a caregiver pick up a child and forcefully sit a child down on a teacher chair with the teacher.

A video was observed showing one child biting another child. The caregivers were not able to intervene because they were on their phones.

Observed on live camera stream that 3 caregivers were on their phones during naptime while caring for children. This was corrected when licensing spoke with the caregivers observed on their phones and the phones were put away.

During the investigation, it was found that a caregiver did not demonstrate good judgment or self-control when the caregiver held onto a child by one arm when the child tried to drop to the floor and then set the child roughly in a chair.

During the investigation, it was found that a caregiver handled a child roughly instead of interacting with the child in a positive manner.
A 5 month old did not have any monthly feeding records in her folder. Infant was enrolled in September 2024.
Obsesrved an infant sitting in a high chair without straps being used. The straps were behind the infant. The child was straped in during the inspection.
Observed a infant sleeping in a crib with a stuffed animal small blankie and burp cloth in the crib. Both of the items were removed from the crib during the inspection.
A review of the Employees Who Left Employment in 2023 data from January 18, 2024, revealed the center had not reported the number of employees who left employment in 2023 by January 15, 2023, as required.
Observed one caregiver who did not have preservice training completed. They are past their 90 days and only completed 5 hours.
Observed 5 staff files, 2 staff were missing photo ID's, 3 were missing proof of education, and one was missing a notarized affidavit.
The infant room was using the same sink to wash hands and to wash bottles. I spoke to the caregivers and directors about cross contamination.
Observed the last gas inspection done in 5/2021 and is needing to be completed again in 5/2023
A child in care was not supervised appropriately by the caregiver, resulting in the child being kicked, hit and stepped on by other children in care.
During an investigation it was found that caregivers do not feel they are properly trained on children with disabilities.
During a DFPS investigation it was found that a caregiver used discipline that was not appropriate to the child's level of understanding.
During an DFPS investigation, it was discovered that a caregiver used prohibited punishment and the operation did not report the incident to licensing.
Observed a caregiver in the bathroom changing diapers, another child was in the hallway under the small slide. The caregiver could not visably see the child as there is a wall between where the caregiver was and the child. The child had access to the hallways which lead to outside and other parts of the operation. This was corrected when the child went back into the bathroom.
Observed on video a caregiver grabbing a child by the ankle and pulling the child back to their mat.
There were two caregivers with conditions on their background checks that stated that they could not be left alone with the children in care, and both caregivers had been caring for children without another cleared background checked adult in the classroom. This has been corrected as both caregivers no longer work at the operation.
Observed a caregivers file and the operaiton was not able to provide a copy of supervsion training for this caregiver
It was found that caregivers did not have specific trainings for supervision or preservice training. This was corrected, as the caregiver no longer works at the operation.
Observed a caregiver not supervising children properly. The caregiver was observed to be speaking with a parent when a book shelf fell on a child in care. Another child had to tell the caregiver what had happened before the bookshelf was taken off the child.
There was no incident report for review.
A caregiver administered medication to a child without washing their hands first. This was corrected when I spoke to the director/person in charge about proper handwashing.
It was found that a classroom of 2 year old children had 13 children and one caregiver when the injury to a child occurred. This was corrected when specified age groups were demonstrated to the caregiver who has mixed aged children.
During a walk-through of the childcare facility, a 2-month-old infant was observed swaddled in a crib. There was no Infant Sleep Exception/Health Care Professional Recommendation form on file for review.
During a walk-through of the childcare facility, infant bottles and a bowl was observed sitting in the hand-washing sink. The infant bottles and bowl was immediately removed from the hand-washing sink when addressed, bringing the facility into compliance.
Based on information obtained during an investigation, it was found that a form of prohibited punishment was used with a child in care during naptime when a caregiver placed a blanket over a child's face and brushed the child's eyes resulting in the caregiver poking the child's eye.
A child was observed sleeping with a training cup during naptime. The training cup was taken away from the child during the inspection.
The School Age classroom was observed watching a movie and playing on their tablets during rest time. The caregiver turned off television and picked up the tablets during the inspection.
It was determined the operation did not notify Child Care Regulation of the air conditioner unit that stopped working, which effected 2 classrooms at the operation.
The operation had a portable air conditioning unit that had a bucket where the water drained into that was accessible to the children in care.
Three non-walking infants younger than 12 months of age were observed to be asleep on boppy pillows on the floor rather than in their cribs. All infants were immediately placed in their cribs when addressed.
A child was observed to be in a restroom down a hallway, alone, and away from the classroom. A caregiver left the classroom and located the child. Appropriate visual and/or auditory awareness was not maintained at this time.
