An infant was not given a bottle during the timeframe that was noted on the infant feeding instructions. The infant feeding instructions stated that the infant needs to be fed every 2-3 hours and the infant was given a bottle after 4 hours.

A child was observed on video being placed on the floor, hitting their back against the wall in an aggressive manner.
Caregiver appeared by video to place a child down at a chair using more than normal force. Caregiver appeared to be flustered and not have control of their actions. Caregiver was terminated by operation.

Observed a 2 month old infant sitting in a boncy seat while the caregiver sat beside the infant holding the infants bottle. This was corrected when the caregiver was told that infants under 6 months old must be held to be fed and the infant was picked up.
Observed two caregivers with access to the children who did not have a cleared background check at this operation. This was corrected when the caregivers were asked to leave the operation and a background check was put it.
Observed dirty diapers on the floor of the bathroom. This was corrected when the diapers where thrown in the trash,
