A review of the Employees Who Left Employment in 2024 Survey data from January 17, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.
In the infant classroom one caregiver was caring for 5 infants ages, 3 months, 3 months, 5 months, 5 months and 6 months. The classrom was over ratio by one infant. The infant was moved to the other infant classroom bring the classroom back into ratio.
Based on the information during the investigation, there is sufficient evidence to support the caregiver using their personal device while supervising children.
Medications in the center were labeled with the children's names, but not the date brought into the center.
Two medications were for children no longer enrolled in the operation. This was corrected during the inspection when the medications were discarded.
The toddlers in the Toddler B room were watching a sing and dance music show on the television. The children in this room are 17 to 19 months old.

Based on the preponderance of information obtained during the investigation a person in charge of the operation was multi-tasking while caring for children. The caregiver left the room multiple times, and was handling other duties, which took their attention away from maintaining supervision. As a result, a child was able to exit the doors of the operation.
