The walls in the operation have both large and small areas where the paint is chipped in multiple places. The walls need to be cleaned, and the window tint in the front of the toddler room is peeling off the window.

Based on the information obtained during the DFPS investigation, a caregiver was seen picking infants up by their arms from the changing table and swinging the infants to their back.

4 children not eating were seated in a high chair for more than 15 minutes with no activity. This was corrected at inspection when the children were removed and activty provided.
