At time of inspection in one room one caregiver had 19 children. The ages ranged from 11 months to 10 years old.

During inspection, children were observed watching television. Based on daily schedule the children were supposed to be participating in table top activities.

At time of inspection, I observed a child open a locked door for an unknown person. Both caregiver and director were located in two separate rooms and could not see who was at the door. Caregiver also left the children unsupervised to open the door on two occasions.

At time of inspection, children were observed watching television for over an hour.

Two out of the three vans observed were due for service in August 2024.

At time of inspection, three vans observed did not have a list of children being transported, medical emergency information for each child, parent contact information.

After two infants were observed done eating the infants were observed sitting in a high chair for an additional 15 minutes.

One out of the three van's child safety alarm did not work.

At time of inspection, the children were observed watching a movie called Hotel Transylvania.

A review of the Employees Who Left Employment in 2024 Survey data from January 17, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

One infant did not have an updated feeding sheet.

Children enrolled on 9/1/23, 9/19/23, 11/1/23 , are missing the emergency contact address on the admission form.

Infants ages 3 months to 15 months old were watching a musical cartoon.

The operation has not notified parents of the Parents Rights as required by 9/30/23.

The toddler and preschool classroom did not have a sign in sheet.

The posted daily menu did not have the posted substitutions for the day. Note: This was corrected during inspection.

Two play yards located in the infant classroom did not contain a flat mattress that snugly fits.

A play yard was observed with a hole in the mesh. SN: Pictures were taken.

Two infants were observed sitting at the lunch table without the proper straps. SN: This was corrected during inspection.

Electrical outlets in the toddler and preschool class were observed without covers. SN: This was corrected during inspection.

The diaper trash can in the toddler restroom was observed uncovered. SN: This was corrected during the inspection.

Two vans did not have the required emergency medical information for students.

Infants ages 7 mo to 16 mo were observed watching television.

A child was sleeping with his bottle. This was corrected at inspection when it was removed.

Cribs in the infant room were not labeled. This was corrected at inspection when cribs were labeled.

Infant daily reports were not completed for the day.
