An infant was observed awake in a crib, in a sleep sack swaddle. The swaddle type was wrapped around the childs torso. The sleep sack swaddle was removed off of the infant.
The 4 year old and older playground does not have the required 6 inches of mulch. Some areas have 3 inches and one area has 5 inches.
Two staff were present at the operation caring for children in classrooms, and did not have an active background check on file.The backgrounds checks were ran while at the operation and the two staff were sent home.

Based on evidence obtained, children are not supervised adequately on the playground resulting in an injury that required medical attention. Observations made during the investigation inspection showed caregivers not intervening as children inappropriately used playground equipment.
