Based on the information reviewed during the inspection a caregiver does not have previous experience and has no pre-service training.

One caregiver does not have CPR/First Aid on file and two caregivers CPR/First Aid is expired.

The operation does not have access to children's files for the 2024-2025 school year.

Based on the documentation reviewed during the inspection multiple caregivers have not completed their annual training.

Two caregivers did not know the number of children they were caring for.

The operation does not have a food allergies list posted or in licensing binder.

Based on information viewed during inspection and information gathered inspection, it determined that the program director and site director are not following the conditions of the waiver/variance that was granted to the operation in January of 2024.

One child's food allergy plan was not signed by a doctor and three children did not have a food allergy emergency plan on file.

The site director did not have immediate access to the children's admission information during my visit. The program director called the site director and walked him through how to access the information through the drive.

I observed 43 children at the operation with 3 caregivers. This exceeds the 35 children maximum group size with 2 or more caregivers by 8 children.

The director designated at the operation on 12/1/2021 is not qualified as they only have 6.5 hours of training.

The old site director was no longer employed as a site director for this operation as of November 30, 2022. A new site director was hired by December 1, 2022. Licensing was not notified of either change within 5 days as required.

2 caregivers are missing all or some trainings in the required health and safety topics

2 medications are missing the date the medication was brought to the operation. 1 medication was missing the childs full name.

10 of 10 childrens files I reviewed are missing addresses for emergency contacts.

1 child had an inidcated allergy without a completed food allergy emergency plan.

10 of 10 childrens records are missing vaccination records. There is also not a statement saying the records are on file with the school.
