Upon arrival at the operation there was one caregivere caring for 26 children ages 6m-5years at nap time. This was corrected within 30 minutes when the other caregivers arrived from lunch and for their afternoon shifts.
An infant was observed sleeping in a restrictive device at nap.
During inspection it was observed that a caregiver left 6 children alone in a room ages 11m-2 years, SAG of 17m, in high chairs when the caregiver left the room to go to their car. This was corrected when the caregiver returned to the room.

Immunization records were reviewed for the HASA. Two records were not current and up to date.

During inspection Lysol wipes were obsesrved to be on a low shelf in the bathroom that the children use and have access to. This was corrected at inspection.
During inspection it was observed that there were only large play items availble for the children to play with in the room and accessible.
The operational policies were reviewed and were missing the activity portion, specifically the benefits of physical activity, duration of play and where the children will go in the event of extreme weather.
The infant feeding instructions were reviewed and have not been updated in the last 30 days. The last date of review was on 4/1/22
Four children files were reviewed for the HASA and one file did not have a current immunization record.
The grounds checklist were observed for the HASA and had not been completed since February of 2022.
The activity plans were reviewed and there is only one outdoor time listed for the children.
An employee was observed cleaning an infants hands after a diaper change with a baby wipe. This was corrected when the caregiver washed the infants hands with water and soap.
This standard was reviewed in relation to the DFPS investigation and found to be deficient. One substitute present at the operation did not have a complete orientation before being counted in child/caregiver ratio.
