The use zone for the rocking piece of equipment did not have a 6 foot use zone.

Upon arrival at the operation there was one caregivere caring for 26 children ages 6m-5years at nap time. This was corrected within 30 minutes when the other caregivers arrived from lunch and for their afternoon shifts.

An infant was observed sleeping in a restrictive device at nap.

During inspection it was observed that a caregiver left 6 children alone in a room ages 11m-2 years, SAG of 17m, in high chairs when the caregiver left the room to go to their car. This was corrected when the caregiver returned to the room.

During inspection the infant daily reports were reviewed and did not have a spot for the summary of daily activities.

During inspection it was observed that the infant play yards did not have a name for each child on the cribs.

During inspection the two infant rooms were out of ratio. One infant room had 1 caregiver with 5 infants 3m-11m. The second infant room had 1 caregiver for 6 children SAG 17m. Both room ratios were corrected during inspection.

Immunization records were reviewed for the HASA. Two records were not current and up to date.

An infant was observed to be unbuckled in the highchair with the tray detached. The child was attempting to climb out of the highchair.

During inspection one classroom was observed to be over ratio while waiting for two caregivers to arrive for work. This was corrected during inspection when the caregivers arrived.

During inspection, one classroom was over ratio. The 2 - 3 yrs old class had 16 children.

During a follow up inspection the operation was operating with 44 children present and capacity is 38 children for the operation total.

During follow up inspection, 1 infant was observed asleep unbuckled in a high chair.

During follow up inspection, 1 infant was observed asleep unbuckled in a high chair, 3 infants were observed in high chairs unbuckled.

A follow up was conducted on this day. One of the children present did not have an updated feeding instruction. The standard was re-cited.

During a follow up inspection the operation was operating with 41 children present and capacity is 38 children for the operation total.

Training records were reviewed for the HASA and one record was missing training for SIDS and Shaken Baby.

Traning was reviewed for the HASA and one employee did not have child maltreatment training as required.

During inspection Lysol wipes were obsesrved to be on a low shelf in the bathroom that the children use and have access to. This was corrected at inspection.

During inspection it was observed that there were only large play items availble for the children to play with in the room and accessible.

The operational policies were reviewed and were missing the activity portion, specifically the benefits of physical activity, duration of play and where the children will go in the event of extreme weather.

The infant feeding instructions were reviewed and have not been updated in the last 30 days. The last date of review was on 4/1/22

Four children files were reviewed for the HASA and one file did not have a current immunization record.

The grounds checklist were observed for the HASA and had not been completed since February of 2022.

The activity plans were reviewed and there is only one outdoor time listed for the children.

An employee was observed cleaning an infants hands after a diaper change with a baby wipe. This was corrected when the caregiver washed the infants hands with water and soap.

This standard was reviewed in relation to the DFPS investigation and found to be deficient. One substitute present at the operation did not have a complete orientation before being counted in child/caregiver ratio.

The CO2 detectors have not been tested and documented since March 2021.

The smoke detectors have not been tested and documented since March of 2021.

The operation has not had a fire drill since May 2021.

During inspection the infant room was out of ratio. There was one teacher supervising 11 children during nap time.

This standard was reviewed in relation to the reported allegations and found to be deficient. The caregiver made a treat of corporal punishment to the child that they might receive at home.
