During a review of staff records, it was observed only two staff members with current CPR training.

Based on the information obtain during interviews and observation, a caregiver was left supervising children alone which is against their current background check conditions.

It was found that Pre-Service for the employee in question, had not been completed within the required timeframe. The employee was counted in child/caregiver ratio prior to receiving their pre-service training hours.

Based on recent supervision concerns, it has been determined that the Director is not ensuring that the operation is operating in compliance. Plans were not put in place to ensure compliance, following recent incidents.

Supervision was not met, when a young child was found in a classroom alone. The caregiver did not have physical and auditory or visual awareness of the child's on-going activity, as the caregiver was on the playground with their class.

During an inspection of the building, it was observed by licensing a hole in the closet door in the gym area.

Two children with documented food allergies did not have food allergy emergency plans.

The activity plan posted for the infants did not include outside time for the infants. One infant room did not have a schedule posted.

The feeding instructions in the infant 1 room were reviewed and have not been updated in the last 30 days.

During inspection there was a toilet lid that was observed in a classroom that had a broken tank lid. This was discussed with the operation.

During inspection there was a slide on the toddler playground that did not have the correct use zone area for the two slides. This was discussed with the operation and corrected at inspection.

During inspection there was a piece of equipment under the large slide on the school age playground that was in the use zone of the slide. This was corrected at inspection.

During inspection there were cleaning and sanitizing supplies that were on a cabinet in reach of the children in a classroom. This was discussed with the caregiver and moved during the inspection.

During inspection two classrooms had outlets that did not have covers that were in reach of the children.

During inspection the loose fill under the swings was observed and measured to be at 1 inch. This was discussed with the operation.
