A child in care is being spanked with a hand by the caregiver(s).

There was a large bag full of OTC medications, such as allergy pills, acetominophen, pepto bismol etc on the kitchen counter.

There were no mattress protectors for the foster children's beds

A caregiver admitted to threatening a child with a spoon as discipline.

A resident stated and caregiver corroborated that the caregiver does not intervene when children in care fight. The caregiver has told the children to hit each other back.

In one service plan reviewed, the parents of a child was not given the required notice of the review.

An incident which occurred in the home on Saturday September 4, 2021, however the report to the department was not received until Tuesday September 7, 2021.

The three year old in the home did not have a mattress protector on his bed.

A child's date of birth was not updated in the child's service plan.

Two child files reviewed nor IMPACT did not contain service plans in the required timeframe.

The family had a previous verification certificate that had been emailed to them, but they have been living at this residence for a couple of months and not yet received anything.

Caregivers were heard yelling at children in care.

A caregiver was heard threatening children with removal from their placement as a result of disciplinary issues.

When requested the agency was unable to provide a copy of child medical records.

The child caregivers did not receive a copy or summary of the initial service plan.

The plan shows that the caregivers were not involved in planning.

The child's plan was developed 1/19/2021. At inspection 3/16/2021, the caregivers did not have a copy of the child's plan.
