Two out of six children admission forms reviewed were lacking the doctor name and address under the health statement.

Two out of six children records were lacking an updated immunization record.

During the investigation inspection it was observed the center did not fill out the infant daily report and provide it to parents at the time of pick up.

A classroom with children between 2-4 years old was observed to be out of ratio. The specified age group for the classroom was 2 years old with one caregiver and there were a total of 15 children. The classroom was out of ratio by 4 children.

The center was cited for ratios three times. The director is failing to ensure the classrooms are within ratio.

The personnel files were not at the center. Corrected at inspection: A person had to bring the files to the center.

A caregiver observed to be supervising child alone did not have first aid training.

The toddler classroom did not have any working lights on besides the bathroom light. Corrected at inspection: They moved the children into another room that had working lights.

A caregiver observed in a classroom supervising children alone does not have CPR training.

The toddler classroom had a total of 14 children in care with the specified age group of 2 years old with one caregiver. The classroom was over ratio by 3 children.

The permit capacity is 52 children and there were 53 children present at the center. Corrected at inspection: A child was sent home during the inspection.

The centers second building had all the lights off including the classroom and hallway lights. Corrected at inspection: The lights were turned on during the inspection.

The center could not provide an emergency safety documentation form that indicated any fire, severe weather, or lock down drills had been conducted.

An old broken diaper changing station was observed in the playground area. A small homemade kitchen play area was observed to be coming apart and have a screw sticking out. Broken and rusted wheels off of a toy were also observed in the playground area. Corrected at inspection: All the broken items were removed from the playground area.

The center could not provide a filled out monthly playground checklist.

A caregiver could not tell me the ages of the children in their care, they did not have a list that had the children's name or date of birth.

Recited: A classroom was observed to have 23 children in care with one caregiver. The youngest in the classroom was 16 months the oldest was 10 years old. The specified age group was 4 years old.

There were 6 infants in the classroom and there are only 4 cribs in the classroom. Corrected at inspection: They add pack-n-play's into the classroom.

A walker was observed in an infant classroom. Corrected at inspection: It was removed from the classroom.

A 4 month old child was observed in a classroom with a 4 year old. Corrected at inspection: They separated the classroom.

A class was observed watching a cartoon and there were infants in the classroom. Corrected at inspection: They turned off the television and moved the infants into another classroom.

The CPSC certificate was not signed or posted.

An infant room did not have a sink.
