During the inspection, a child was observed alone in a classroom. I observed the caregiver go to the classroom with the infant when I arrived.

A caregiver was observed to have wiped an infant's face and then observed utilizing the same wipe to wipe another child's face. Proper hand washing was reviewed with the caregiver and director during the inspection.

The operation had not completed the repairs needed in the outdoor play area. In the outdoor playground area, the wooden fence had an opening between the planks that were observed to be warped. The linked fence was observed to be coming down in section and there was an exposed telephone wire.

Upon arrival the director left her infant classroom unsupervised when she opened the door.

There were 3 children that were grouped together during naptime and appropriate spacing of the mats was not observed. There was no walk space between each mat.

During the current inspection, there was not another present in the building, during naptime.

As I entered the infant classroom, the classroom was observed to be dark and the director had to open the curtain to allow light in the classroom.

A 3 month old infant was observed napping with a blanket in the crib. The caregiver removed the blanket from the crib during the inspection.

In the infant classroom, there was a 3 month old and three 2 year olds, which presented over an 18 months age gap. Another caregiver arrived to bring the classroom into compliance.

There were 3 outlets that were observed without an outlet cover. The Director covered the outlets during the inspection.

A school age child was observed walking out of his classroom and going to the director's office unsupervised. Upon arrival the director left her classroom unsupervised when she opened the door.

In the infant classroom, the caregiver had 6 children (3 months, 10 months, 15 months and 3 2 year olds), the classroom was over ratio by 1. An additional caregiver arrived to bring the classroom into compliance.

A tube of neosporin was observed accessible to children in care.

Soiled diaper with feces was observed accessible to children in a trashcan in the kitchen area. The trashcan was observed to not have a lid and the diapers were not wrapped in a bag.

A caregiver was unaware of the ages of children in her group.

The operation was observed to be cluttered and food debris was observed on the floor in the main area and back classroom.

The operation did not have documentation of fire drills being completed for the year.

A child in care was observed napping in front of the back door that leads to the playground. The caregiver placed the child in another spot during naptime.

There were 2 infants observed in a high chair that is equipped with safety straps and the safety straps were observed to not be fastened.

The nap room where the 3 to 5 year old children were sleeping was observed to have been to dark. A flashlight had to be turned on to count all the children in the classroom.

There was a 2 year old observed in the Infant Room with the youngest infant, 4 months of age.

There were 2 infants in care that did not have an infant feeding instructions on file for review.

During naptime, there was not another caregiver observed in the building, that was not counted in ratio.

One child in care did not have a health statement on file for review.

A 19 moth old child was observed sleeping with a bottle during nap time. The caregiver was observed to have removed the bottle.

Three infants in care, (4 month, 7 month, 8 months old) did not have update infant feeding instructions on file for review.

The operation had debris on the floor (crumbs, trash) and blankets. Under a changing table, there was dirt and crumbs.The trashcans did not have lids and were observed to be full. The Director picked up the blankets, swept the floors and cleaned under the changing table during the inspection. The Director placed a lid on the trashcans and removed the trash bags.

A 4 month old infant was observed sleeping in a restrictive device (bouncer). The Director removed the infant from the bouncer.

An 8 month old infant was observed asleep in a crib in the infant room without a caregiver present in the classroom. A 4 year old child was observed going to the restroom towards the back of the building without a caregiver present to supervise.

There was 1 caregiver present in the building during nap time with 17 children in care (4 months to 4 years old). Two other caregivers arrived during the inspection.

The Director had not completed the 30 hours of required annual training. The Director had 14.5 hours of annual training on file.

The children in care, ages ranging from 4 months to 4 years old) were observed watching television during naptime.

There were 2 caregivers present in the building each supervising a group during naptime and there was not another caregiver available at the operation.
