A 5 month old infant was observed in a restrictive device with a bottle being propped up by a blanket to help assist the infant.

The children product certification was last reviewed on 02/07/2024. NOTE: This was corrected when the primary caregiver updated the children product certification.

The operation did not provide for two children, information on if the child has been seen by a physician within a year by the previous due date 12/18/2024.

The operation did not obtain 2 children immunizations that were due on 12/18/2024.

Three out of four children files lacked information on if the child has been seen by a physician within a year.

The primary caregiver was in the breezeway drinking coffee while children were left inside with no supervision.

During an inspection at the home, the primary caregiver was observed to be operating out of compliance with minimum standards when she was visiting with a family member in the breezeway which is not licensed care space while the children were left unsupervised in another area of the home. Primary caregiver has been previously cited for leaving children unattended in this manner resulting in corrective action being imposed by CCR..

During the course of this investigation, it was determined that the primary caregiver left children unsupervised in another part of the home for 51 minutes.

The operation had Clorox Deodorizer on the counter in the bathroom accessible to children. The outdoor area had roundup, potting soil which is labeled to be" keep out of the reach of children", cutter yard spray, and mosquito spray in reach to children. The grass was high and needs to be mowed. Multiple nails were sticking out of the fence that need to be hammered in. There is a board missing on the fence that needs to be replaced. The storage building door was open and accessible to children. The garbage can in the restroom was overflowing with pampers.

The operation has a large trampoline in the backyard accessible to children. .

Screen time was being utilized with two children that were under two years old.

The children did not wash their hands before eating.

The caregiver did not wash her hands before handling and serving food to children..

The operation did not update the Children product certification since 11-27-2017.

RECITE: During the course of the investigation it was determined that the permit holder smokes tobacco cigarettes while children are in care at least twice daily.

Recite: During the course of the investigation, it was determined that the permit holder is not properly supervising children in care due to her practice of going outside to smoke cigarettes while children are left alone indoors at least two times daily. A keypad lock has been installed on the front door to help secure the home.

There were a total of seven children in care. The operation was over child/caregiver ratio by one child.

It was reported that the permit holder withholds snack from a child when they have potty accidents.

During the course of the investigation it was determined that the permit holder smokes tobacco cigarettes while children are in care at least twice daily.

During the course of the investigation, it was determined that the permit holder is not properly supervising children in care due to her practice of going outside to smoke cigarettes while children are left alone indoors at least two times daily. It was determined that the permit holder was unaware of a child being picked up by their parent due to being in the restroom assisting another child until she returned to the main care location and was told by other children where the child had gone. She did not have adequate auditory or visual awareness of what was happening for at least five minutes during the reported incident.
