During the course of an investigation, it was determined that a caregiver throws children's food away while they are still eating.

During the course of an investigation, it was determined that a caregiver is yelling at children.

The operation lacked the following operational policies- 746.501(18)and 746.501(28).

The toddler room had windex accesible to children.

The first aid kit had multiple items that were expired. NOTE: This was corrected at inspection

The toddler room diaper changing pad was torn.

A wooden post with nails exposed was observed on the playground, posing a risk to children. Note: This was corrected when the director removed the wooden post from the playground.

Operation placed a child younger than 12 months of age in a playard with a blanket.Two six month old infants were observed sleeping in their playard with blankets covering their body.

Two personnel records reviewed did not have the complete 24 hours of annual training complete. One caregiver has completed 10 hours and another caregiver has completed 16 hours of annual training.

Through information received with this investigation, it was determined that the caregiver was rough in grabbing a child for his failure to comply with her verbal prompts.

Based on information received from staff and children, the caregiver failed to demonstrate good judgement when she was rough in grabbing children to make them comply with her.
