The director did not have current training in transportation.

The operation did not conduct fire drills for the month of June and July. NOTE: This was corrected at the inspection when the operation performed a fire drill under 3 minutes.

A school age room had 38 children in care and their ages were 8-12 years old. There were two caregivers present. The maximum group size is 35 children for that age group. The operation was over ratio by 3 children.

The operation allowed a person that was listed as ineligible, to be present and counted in ratio at the operation.

Three electrical outlets were not covered. Note: This was corrected at inspection when the operation placed outlet covers in the outlets.

The operation did not conduct a fire drill for the month of August. NOTE: This was corrected at inspection when the operation conducted a fire drill.

Three first aid kits were not complete in the vehicles that transport children.

While reviewing files two caregivers did not complete their required 24 hours of annual training. One had a total of 11 hours, 13 additional hours needed. One had a total of 14 hours, 10 additional hours needed.

The director did not have verification of her required 30 hours of annual training. Total training hours met = 13, 17 additional hours needed. There was no verification of the additional 2 hours of required transportation training needed for drivers.

A caregiver had an expired CPR/First aid certification in her file.

Two personnel records reviewed did not have 24 hours of annual training complete at the time of inspection.

I observed the fire extinguisher in the transportation van to be expired; last serviced October 2020.

Operation allowed one caregiver to be present without a cleared background check prior to supervising a group of children.
