During the inspection the incident reports for children involved in an vehicle accident was not available for review during the inspection.
Based on the information gathered during the investigation it was determined that the operation vehicle was involved in a accident and one child received an injury that was evaluated by a medical profession. This was corrected during the inspection by explaining to the director the reasons and how to report injuries/incidents to licensing.
Based on the information gathered during the inspection it was found that parents of the children involved in vehicle accident did not received a signed copy of a incident report.
