An infant was observed sleeping on an elevating medical pillow with straps due to having reflux. The infant caregivers stated that two infants use these pillows and that they also use them in their cribs. The infants had doctor's notes but no Infant Sleep Exception forms on file.

During the investigation inspection it was found that both caregivers left the classroom after an incident between two children had occurred, leaving the classroom of children without a caregiver for a few seconds.
An incident occurred between two children in care where the two children began to fight and hit one another. One of the children was hit with a metal bell before a caregiver intervened to stop the children from fighting. One of the children required stitches due to this incident.
Observed the same toy being place in mouth by different children multiple times without being set aside to sanitize. Toys were placed back in toy bin with clean toys. This was corrected by the toys being taken out and set aside to be sanitized.
