During current inspection, there was a broken toys with sharp edges observed in the playground the children were in. The caregiver removed the toy during the inspection.
A caregiver informed she had 4 children in care and it was determined she had 3 children in care. The children were all grouped together during outdoor play.
Based on the evidence from the DFPS investigation, a child in care received an injury that required medical attention, and the caregiver did not provide first aid.

During the inspection, I observed a caregiver leave her classroom to adjacent classroom. There was not another caregiver supervising her children. In another incident, it was determined a child was able to crawl under a fenced door and get into an area that is not regulated. The area had hazards such as cleaning supplies, tools that could have been a safety risk for the child. The child was able to be removed from the area.
The operation does not have a permit to serve hot meals and was observed heating corn and ravioli for lunch. The caregiver ordered pizza for lunch during the inspection.
The operation's first aid kit was missing a thermometer, tape, antiseptic wipes and gloves.
The operation did not complete any fire drills for the months of November or December 2024. The operation only completed 3 shelter, severe weather and lock down drills for 2024.
There was a room the children eat lunch in that did not have a smoke detector.
The operation did not test the smoke detector, carbon monoxide or inspect the fire extinguishers for the month of December 2024. The smoke detectors and carbon monoxide detectors were tested during the inspection. The fire extinguishers were observed to be within the proper fill line during the inspection.
A 13 month old infant was observed a lone in a classroom during the inspection.

There were 4 toddlers observed sleeping with a sippy cup. The caregiver removed the cups during the inspection.
