There was an infant asleep on a boppy pillow on the floor in the infant room. This was corrected during the inspection when the infant was moved to a crib.

In one of the toddler classrooms, there was a bottle of multi-purpose cleaner sitting on a cabinet within the reach of children. This was corrected during the inspection when the cleaner was moved to a counter out of the children's reach.

There was sufficient evidence found during the investigation to show that caregivers did not provide an adequate level of supervision to intervene and ensure the safety of children on the playground.

One classroom of 5 infants, ages 10 - 13 months, did not have their hands washed upon returning inside from outdoor play.

One infant was observed to be swaddled in a bare crib during the inspection. The swaddle was removed.

It was observed that the fence on the playground was not secured properly. Children were observed lifting the bottom of the chain link fence posing a safety risk.

There was sufficient evidence found during the investigation to show that prohibited punishment was used when children were pulled or moved aggressively by the arm.

There was sufficient evidence found during the investigation to show that the minimum director presence of 75% was not being met.

There was sufficient evidence found during the investigation to show that children were served undercooked or overcooked food at the operation on multiple occasions.

There was sufficient evidence found during the investigation to show that caregivers used prohibited punishment when yelling at children.

Children under the age of 4 years old were observed to be served whole hotdogs during lunch. The food was cut into small peices during the inspection.

One infant under 12 months old was observed sleeping in a crib with a stuffed animal. The toy was removed during the inspeciton.

There was sufficient evidence found during the investigation to show that the safety straps on one highchair were not fastened, causing a child to fall.

There was sufficient evidence found during the investigation to show that infants were placed in cribs for reasons not related to sleep and when discontent.

There was sufficient evidence found during the investigation to show that caregivers were completing janitorial duties while responsible for the supervision of children.

Two caregivers did not wash the hands of two infants after diapering the children.

One caregiver was observed wiping the noses of different children without washing their hands in between.

One caregiver in a 12-17 month old classroom did not wash a child's hands after diapering the child.

Four infants were observed eating in highchairs without the safety belts fastened. This was brought into compliance when the infants were buckled in their seats.

Three caregivers were found to not know how many children they were responsible for during the inspection.

Four infants were observed in highchairs without safety straps buckled. This was corrected when the caregivers buckled the straps during the inspection.

Two caregivers stated that they had a combined total of 33 children during naptime. The caregivers were observed to be responsible for 35 children.

During the investigation, it was found that classroom control was not being maintained in one classroom.

The activity plan for one classroom was observed to be outdated and there was no current activity plan being followed by caregivers.
