The operation did not have a signed children's product certification form verifying that the director had reviewed the children's product recall list. This was corrected during the inspection when the director confirmed that she is subscribed to the recall emails and signed a certification form.

The first aid kit was missing tweezers, gloves, and a first aid guide.

There were no playground maintenance checklists available for review. This was corrected during the inspection when the director conducted a check of the playground and completed a playground maintenance checklist.

Out of the four staff files reviewed, one caregiver was missing 15 out of 24 hours of annual training, one was missing training in preventing emergencies due to allergies, one was missing training in preventing abuse and neglect, and one was missing training in preventing abuse and neglect, SIDS/Shaken Baby Syndrome, early childhood brain development, emergency preparedness, preventing emergencies due to allergies, building and physical premises safety, and handling hazardous materials.

There were a bottle of Clorox wipes sitting on a shelf within the reach of children. This was corrected during the inspection when the wipes were moved to a shelf out of the reach of children.

One infant's written feeding instructions had not been updated since March.

Out of the four children's records reviewed, two were missing health statements, two needed updated immunization records, and one was missing hearing and vision screening results.

Out of the four staff files reviewed, four were missing proof of background check requests, one was missing a photo ID, and one was missing a copy of her GED.

Uncovered outlets were observed in the pre-k room. This was corrected during the inspection when outlet covers were placed on the outlets.

There was a slide under five feet tall with no loose-fill in the use zones.

A child was sent to the bathroom without being accompanied by a caregiver. This was corrected during the inspection when the child returned to the classroom and the caregiver was informed that children cannot be sent to the bathroom alone.

During the inspection there was no documentation of a infant caregiver having the required pre-service trainings for caring for children under 24 months.

During the inspection the last fire drill was documented in March 2021. This was corrected during the inspection by a fire drill being condcuted.

A slide and a rocking plane was observed on the playground without loose fill.
