I observed 27 preschool and school-age children (ages 3-9 years) in one classroom with one caregiver, exceeding the child/caregiver ratio of one caregiver for this age group.
I observed 34 toddlers (ages 18 months-2 years) in one classroom with 2 caregivers, exceeding the maximum group size for this age group.
It was determined that parents were not provided written notice regarding a Hand, Foot, and Mouth disease outbreak.
