There was mold growing on the bathroom ceiling, the window mini blind, and the air vent. Bugs were observed in the bathroom which did not have installed window screens. Also, it was noticed that a child's bedroom did not have screens installed. Pictures were taken.

It was determined a home screening from a previous agency was used. A new home screening was not conducted.

A 2 year old child sustained an injury to his neck and legs by an object.

A caregiver hit a foster child with a belt or an object as a form of discipline and the child received injuries.
