Based on information obtained through investigation, it was found that caregivers in the operation heard an infant crying and came to the classroom to comfort him while his assigned caregiver was present multiple times.

Based on information obtained through investigation, it was found a caregiver had withheld information about how a child was injured in care. This information was not provided to the operation until later the same day, after the caregiver had left and the child had been picked up and taken to seek medical care.
