During this inspection, there were two staff member laying on the floor in the infant room. One staff member appeared to be asleep and was woke up by another staff member. Upon review of the video footage of the room, that infant teacher could be observed on camera appearing to fall asleep from 1:26pm to when I walked into the room around 1:47pm.

During this inspection, I again observed an infant teacher to be using their phone for personal use instead of activily supervising children in their care. Upon review of the video footage of the room, another infant teacher was also on her phone for personal use prior to my entry into the room.

During this inspection, I observed an infant teacher to be using their phone for personal use instead of activily supervising children in their care.
During inspection of a 18-24 month group of children, screentime was being used on a tablet attached to the wall. This was corrected at inspection when it was turned off.
It was determined that a child in care was left unattended in a vehicle owned and operated by the operation.
It was determined during the investigation that the vehicle used to transport the children in care did not have an electronic child safety alarm installed.
The incident report was not signed by the parent.
It was determined throughout the course of the DFPS Investigation that a child in care was neglectfully supervised.
It was determine during the course of the investigation that a caregiver did use their phones for personal use while working in the classroom with children.
A room with 7 children, ages 12-18 months, was observed to have one caregiver supervising children in a reduced ratio classroom. This was corrected at inspection when another caregiver entered the room.
The operation was lacking a current fire inspection.
An infant was in a bouncer for a period of at least 30 minutes or more. This was stated by the caregiver in the infant room. This was corrected at inspection when the caregiver removed the infant from the restrictive device.
A crib that an infant under the 12 months of age was resting in contained a large toy for the infants entertainment. This was corrected at inspection when a cargiver removed toy from the crib.
All classrooms in the operation were lacking posted activity plans.
A child with a doctors note stating that the child was allergic to citrus/oranges did not have an allergy emergency plan on file.
During lunch time there were 4 infants in the kidney bean shaped toddler table that were not buckled in.
Several of the infant playyards contained matresses that were not designed by the manufactuer, causing the mattress to not fit snugly in the playyard. This was corrected at inspection when the mattresses were removed from the playyards. One infant crib had a loose fitted sheet causing the sheet to bunch up on the mattress. This was corrected at inspection when the sheet was changed to a snug fitted sheet.
It was determined that a child in care was left unattended in a vehicle owned and operated by the operation.
There was a hole in the wall in the 2 year old room, Fans in the classrooms need to be dusted. A sink in the afterschool room needs repair. The seats on the dining table were leaning and need to be leveled. There was standing water in the pots by the front door. Outlet covers were missing in several rooms, these were replaced during the inspection.
