A child's record was missing a health statement from a medical professional.

A caregiver was observed on video footage to have aggressively carried a 1yo child by the arm.

Based on the information obtained from the DFPS abuse/neglect investigation, a caregiver was found to have neglected to protect children being physically abused by a fellow caregiver by failing to report the abuse as mandated.

Based on the information obtained from the DFPS abuse/neglect investigation, it was found that the director failed to appropriately supervise caregivers to ensure that children were not abused/neglected.

Based on the information obtained from the DFPS abuse/neglect investigation, it was found that two caregivers were being unnecessarily harsh and cruel in their treatment of children. Caregivers were observed to forcefully grab and pull children's arms, grabbing children by their clothing in order to force them to sit down, grabbing a child by their shoulder causing them to fall down, and were also observed to have dragged a child across the daycare room floor.

Based on the information obtained from the DFPS abuse/neglect investigation, two caregivers were found to have physically abused children in care and one of which was found to have additionally neglected to protect the children by failure to report that children were being abused.

Based on the information obtained from the DFPS abuse/neglect investigation, it was found that two caregivers were being unnecessarily harsh and cruel in their treatment of children during nap time. Caregivers were observed to pick children up and slam them onto nap mats, pulling children by the arms to force them to lay down straight, kick a child on their legs and back to recenter their bodies on the nap mat, dragging children to the nap mat, and shoving children face down into the nap mat.

Four caregivers in four different classrooms were observed not to have washed a child's hands following diaper changes.

A caregiver in the infant room was observed to not wash their hands following a diaper change.

Three caregivers in three different classrooms were observed not to have washed a child's hands following diaper changes.

A caregiver in the infant room was found to have had their personal cell phone out with the text messaging screen on indicating that it was being used for personal use while also playing with an infant in the floor.

A child was placed in a high chair/restrictive device as a form of discipline because the child would bite other children and would also climb on furniture.

Based on the information gathered during the investigation a caregiver did not use good judgment when picking up a child in care.
